ISP
The Pan Hampshire Information Sharing Protocol has been produced by the CAFA 2 Information Governance and Consent Group to demonstrate their commitment to effective and appropriate information sharing.  Organisations signing up to this protocol have all agreed to comply with the national standards within the Information Governance Toolkit and are considered to be a ‘trusted’ organisation.  The protocol enables information sharing to take place between the different organisations but is not a substitute for consent from the individual unless there is a legal basis on which to do this.  Please refer to the protocol for an up to date list of the parties included.

Consent to View and Share
It is best practice for Health and Social Care organisations to ask the individual at the start of care who they would like their information shared with and when they would like to be asked before information is shared.  The Southampton Overview Group, made up of users and carers have produced a Consent to View and Share form to give examples of how this might happen.  The individual can use this form to help them decide who they do and do not wish their information to be shared with.   This gives the individual control over their information. However, where there is a legal requirement to do so, information may be shared without consent.   Most organisations have their own Consent to View and Shared form and this example may help the individual to consider the question of consent and sharing their information ahead of being asked.
