What should you do with your completed forms?
The important thing is that your forms are easily available when needed. You may like to keep a copy of your Personal Profile with you, so it is available in an emergency. You may decide to give copies to family, friends, and people helping to look after you. It’s helpful to keep a copy at home with your important documents. If you keep a copy of your forms on your home computer, and your circumstances change, you will be able make changes to them so your forms are kept up to date.
What’s important is that you can get hold of the information quickly if it’s needed.

When you complete any of the forms on this website, the information will NOT be saved on this web site or any other external site or location. You have the option to save your personal information on your computer, or print it out
However, if you are a resident of Hampshire, Southampton or Portsmouth, you can choose to put your information onto the Hampshire Health Record. This will allow your information to be made available to professionals without delay.
