This website contains forms which give you the opportunity to record key health and social care information about yourself, and also wishes that are important to you.
The Personal Profile is at the heart of these forms. Everyone should complete a Personal Profile and other forms, if they need to.
These other forms include the Advance Care Plan which helps you record wishes you might have and decisions you have made about your advance care, and the Arrangements I Have Made form which details any legal arrangements you have decided about your care and personal life, in case you are unable to communicate these yourself. You can download these forms to either fill in on your own computer, or print off to complete by hand.
The website also has personal care plans designed to help you, and any health care professionals you might be involved with, manage any long term health conditions you might have. They give you information on how to keep yourself as well as possible. These forms cover conditions such as diabetes, chronic obstructive pulmonary disease (COPD), and heart failure. By using these to record information such as blood pressure, weight, and medication, you’ll have information about your condition in an emergency or when seeing unfamiliar doctors. This means the professionals will know what’s normal for you, to help them make the right decisions.
